Astım Nedenleri ve Tedavisi

Astımın kesin sebebi belli değildir. Genel kabul gören görüşe göre, doğuştan soluk boruları duyarlı bireylerde çevresel koşulların etkisi ile astım oluşmaktadır. Astımın tipik karakteristiği bronşların mukoza ödemiyle daralmasından ötürü olan episodik dispnedir (nefes darlığı). Bu durum, polijenik bir kalıtım biçiminde genetik kökenli olabilir; fakat enfeksiyon, alerji ve emosyonel faktörler de rol oynayabilir. Psikolojik mekanizmaların rol oynaması için bronşiyal aşırı duyarlığa somatik bir yatkınlık bulunması gerekir. Astımlılarda belirli birtakım kişilik özelliklerine rastlanmasına rağmen, bu hastalıkla birlikte görülen spesifik bir kişilik tipi yoktur; anksiyetenin yol açtığı bazı astım nöbetlerinin nedeni bir şartlı refleksle açıklanabilir. Kesin alerji vakası gösteren bazı hastalarda bulunan yüksek bir immunglobulin (IgE) seviyesi, alerjinin oynadığı rolü açığa çıkarmıştır. Bu gibi hastaların yeni bulunan immunosüpressif ilaçlarla tedavileri yararlı olabilir. Kortikosteroid'ler de tedavide yararlıdırlar, fakat uzun süre ve yüksek dozda kullanılırsa bazı yan etkileri olabilir. Eğer astım nöbetlerinde şartlı anksiete belirgin bir rol oynuyorsa, hayal gücünde sistematik desensitizasyon yöntemi uygulanabilir. Eğer hipersensitivite dolayısıyla hava yolunun reversibl tıkanmasına yol açan birçok stimüle edici faktörlerin astıma neden olduğu düşünülüyorsa, önce mümkün olduğu kadar açık bir biçimde bu fiziksel ve psikolojik nedenlerin tablosunu çizmek ve bunlardan herbirini tedavi ederken hastayı sürekli kontrol altında tutmak gerekir. Astım sıklıkla bir alerjiye bağlı olmakla beraber (%60-80) alerji olmadan da astım olabilir.. Doğuştan ve çevre faktörlerinden de gelebilir. Eğer derhâl doktora başvurulmazsa(belirtiler nüksettiği anda)ölümcül sonuçlara yol açabilir.

Belirtileri 

Hastalarda zaman zaman hırıltı, nefes darlığı ve öksürük olur. Bazı durumlar astım belirtilerinin çok artmasına sebep olur. Bunlar: mikrobik hastalıklar, soğuk hava, kirli hava, sigara dumanı, alerji yapıcı maddeler (allerjenler), egzersiz ve psikolojik bozukluklardır.
Astımda belirtilerin aniden ortaya çıkmasına astım atağı veya astım krizi adı verilir. Bu durumda hastalarda ağır bir nefes darlığı olur.
Astımı belirtilerin şiddetine göre hafif aralıklı, hafif süregen, orta süregen ve ağır süregen olarak sınıflamak mümkündür. Astımın tanısında muayene bulgularının yanı sıra, kanda IgE' nin ve eozinofil adı verilen akyuvarın sayısının yüksek bulunması, solunum testlerinde soluk borusunda daralma olduğunun gösterilmesi ve deri testleri ile hastaların neye karşı alerjisi olduğunun gösterilmesinin çok büyük bir rolü vardır. ayrıca sert nefesler gözükür .

Tedavi

Astım tedavisinde kullanılan bir solunum aleti.

Tedavide kullanılan ilaçlar iki gruba ayrılır. Birinci grup ilaçlara rahatlatıcı ilaçlar adı verilir (salbutamol, terbutalin gibi sempatomimetikler).Bu tür ilaçlara inhaler de denilir.Kriz esnasında veya belirtiler başladığında kişi inhaler'i ağzına götürür ve soluk alma (inspirasyon) sırasında inhaler'i sıkar. İkinci grup ilaçlar astımdaki temel sorun olan hava yolundaki iltihabın azaltılmasına yöneliktir. Bunlar da solunum yoluyla alınan kortikosteroidler, kromolin sodyum, nedokromil sodyum, teofilin ve lökotrien reseptör antagonistleridir.
Alerjik astımlı hastaların bir kısmında ilaç tedavisi ve korunma yöntemleri etkili olmamakta ve aşı tedavisi (immünoterapi) gerekli olmaktadır.
Astım, özellikle çocuklarda hafif bir tablo gösterirse belirtiler (%50-%60) tamamen kaybolabilir. Ancak yetişkin astımlıların belirtileri çoğu kez ömür boyu kalıcıdır.
